This project is a one of a kind in Ontario and represents an innovative partnership model that will provide the greater Orleans community with access to a comprehensive range of primary care programs and services. All services will be coordinated within a unique”care connector concept” that will provide a highly integrated, patient and family focused continuum of care.

The Orleans Health Hub contains a Family Health Team, an Urgent Care Clinic, a laboratory, diagnostic imaging services, geriatric care and mental health services. In addition, the Hub has Community Care Access Centre on-site and collaborate with local resource centres.

ZW Management Inc. provided full Project Management Services for the Orleans Family Health Hub (OHH) and managed the development of the Master Plan and Master Program.
